Important: Medication Forms Needed for School Year 2024-2025

Hello from your SOAR Elementary School Health Team, Derek Begun and Kelly Brown!
We are reaching out in preparation for receiving your student’s daily medication during the 2024-2025 school year. Please disregard this email if your student doesn’t need daily medication during school hours 7:40 AM through 2:30 PM.
If your child will need to be given a daily medication during school hours or needs emergency medication during the school day, please see the attached medication authorization form for your student’s physician to complete. If you turned one in last year we do require that a new form is filled out for this year. If your child has asthma or any allergy that requires an Epi-Pen please reach out to your doctor to receive their Asthma Care Plan or Allergy Care Plan. We cannot administer any medication until we receive all proper documentation from your child’s physician.
If your family needs a physician or healthcare provider, we have a Denver Health Clinic on our campus. They offer a wide selection of free services to the community from vaccinations, routine checkups, and even dental!
The Denver Health Clinic is located on our campus in Building 1.
